What is a MedsCheck?

A MedsCheck involves a one-on-one consultation with your local TerryWhite Chemmart® Pharmacist. They will review your medicines in store to help you:

  • Identify any concerns or problems you may have with your medicines
  • Better manage your health conditions and improve your general wellbeing
  • Improve the effectiveness of your medicines by using them correctly

What is a Diabetes MedsCheck?

A Diabetes MedsCheck involves a one-on-one consultation with your local TerryWhite Chemmart® Pharmacist. They will review your type 2 diabetes management plan in store to help you:

  • Improve your understanding of your diabetes medication
  • Use your blood glucose monitoring devices more effectively
  • Manage your blood glucose levels
  • Understand the best time to test your blood glucose
  • Reduce the risk of developing complications associated with type 2 diabetes

  • Have you recently been discharged from hospital?
  • Are you taking 5 or more prescription medicines?
  • Have you had recent changes to your prescriptions?
  • Do your medicines (e.g blood thinning medicines) need monitoring?

At TerryWhite Chemmart Nelson Bay we can collaborate with your GP to advise you in the correct use of your medicines.

Speak to your GP about your eligibility for this service.

Once you’ve got a referral one of our accredited pharmacists can come to your home and conduct a medicine review.

They can provide you with one on one advice, which can help you:

  • Understand how to take your medicines correctly
  • Learn why and when to take them
  • Know where best to store your medicines
  • Recognise what problems you should report to your GP
  • Make sure your prescription medicines, over the counter medicines and vitamins are appropriate to take with each other
  • Clarify any confusion you have with generic medicines
  • Learn about Medicine Manager, which helps you organise taking your medication
  • Change your medicines (if needed)

Meds Check

A free 20-minute review of your medication conducted by a pharmacist in the pharmacy. Recommendations can be sent to your doctor.

Diabetes Management Check

A free 20-minute consultation with a pharmacist that will focus on the best way to manage blood sugar levels and diabetic medication. Recommendations can be sent to your doctor.

Home Medication Review

A 60-minute consultation with an accredited pharmacist in your own home. This service is free when a referral is received from your doctor. A report is sent to your doctor that will assist the doctor to manage your treatment.

This service is provided free and is subsidised by the Pharmaceutical Benefits Scheme (PBS).
